It doesn't count for much, but just out of curiosity, do you know if the patient received therapeutic hypothermia?

Also, the final ECG is labelled as occurring 24 hrs later when I believe you mean 48.Vince Dhttps://www.blogger.com/profile/10636259293820649555noreply@blogger.comtag:blogger.com,1999:blog-549949223388475481.post-56475634846585092022013-10-31T11:40:35.849-05:002013-10-31T11:40:35.849-05:00Yes! But I must defer to the ones who interpreted the angiogram that it is sufficiently likely to NOT be ACS that they had to place an IVCD.
Steve SmithSteve Smithhttps://www.blogger.com/profile/08027289511840815536noreply@blogger.comtag:blogger.com,1999:blog-549949223388475481.post-30543361057334718712013-10-30T23:36:33.000-05:002013-10-30T23:36:33.000-05:00Thanks for your interesting case. And the progression of cardiac enzymes also? Sometimes, Could we end up with an autolyzed thrombus/Prinzmetal induced STEMI diagnosis?Anonymoushttps://www.blogger.com/profile/00696129566615350737noreply@blogger.comtag:blogger.com,1999:blog-549949223388475481.post-27596442255554188122013-10-29T11:44:13.523-05:002013-10-29T11:44:13.523-05:00Thanks, Charles! From the info I have, I would have thought STEMI also, but I did not see the angiogram and am not an expert at interpreting that, so I can't be certain and must defer to the uncertainty of the cardiologists.Steve Smithhttps://www.blogger.com/profile/08027289511840815536noreply@blogger.comtag:blogger.com,1999:blog-549949223388475481.post-56908982850649025372013-10-29T07:55:06.107-05:002013-10-29T07:55:06.107-05:00Thanks for another fascinating case. What could have happened that made the 9th defibrillation successful, other than reperfusion of LAD? Would be interesting to interrogate the ICD in a year or so for arrhythmias. Must've been some high quality CPR for no neuro defecit with that much down time, 1st responders should be proud. Not sure if she would have qualified for the ICD in the UK, although would clearly be a special case for consideration.
