So this does not help. With the absence of ischemic symptoms and the short QT, it is prudent to get frequent serial EKGs and troponins, and admit the patient for observation. But it is appropriate to NOT treat for ACS.Steve Smithhttps://www.blogger.com/profile/08027289511840815536noreply@blogger.comtag:blogger.com,1999:blog-549949223388475481.post-10508872346053093202011-05-14T08:35:08.990-05:002011-05-14T08:35:08.990-05:00This may be an example of "treat the patient,...This may be an example of "treat the patient, not the EKG", but with the lateral depressions and the ST elevation in aVr in an elderly lady that may not have traditional symptoms of ischemia, what is your level of concern for severe proximal coronary artery stenosis in this patient? Is it the shorter QT interval or are there other things as well?MC, M.D.https://www.blogger.com/profile/01819116023393064278noreply@blogger.comtag:blogger.com,1999:blog-549949223388475481.post-41053108182115048282011-04-24T21:25:02.272-05:002011-04-24T21:25:02.272-05:00I would only record posterior leads if I had a mod...I would only record posterior leads if I had a moderate to high suspicion of ischemia.
